hello
i placed four implants in edentulous mandible anterior region, 4.3 with 15 mm ,but one just failed without any obvious reason ,i removed it and placed pt on some
antibiotics,as there was a periapical infection below the implant in panorex
i want to replace that implant ,please advise how to proceed,
i can post panorex on your website for you guidance ,pt is 65 year old without any systemic cause bone density is D3 i want to give him an i
implant retained overdenture

Unless you're immediately loading, 15mm is too long and increases the risk for sublingual bleeds. The majority of bite force is distributed in the first 5mm in an integrated implant and virtually none after 10mm. The only reason to go longer is if you need primary stability. Keep it easy and place a shorter implant in a 2-stage approach. 2-stage has a higher success rate and eliminates the need for longer (and more dangerous) implants.
