Hip Grafting for dental implant surgery is it really needed anymore?

I had a patient sent to me recently who saw her local oral surgeon where he did bilateral sinus lifts using hip grafts. I thought that was kind of invasive especially when the literature supports the use of Bio-oss. With all the advances in bone grafting is it really needed???

I not only think you do not need hip graft bone for Sinus Lifts but it is actually contra-indicated. Autogenous bone from the hip has a high turnover rate and seems to resorb over time. I utilized this bone graft for Sinus Lifts back in the early to mid 1990's. Although we had success, over the following 10 years of follow up there appeared to be tremendous resorption of the grafted bone in the sinus not immediately next to the implant. My own theory is that autogenous bone not "loaded" by the implant was resorbed. This does not happen with cortical-cancellous allograft or Xenograft materials.
Hip grafts for other edentulous areas, not the sinus, may be effective when over corrected and mixed with a low turnover bone product.

I totally agree with Dr Salama.From my own experience
there is unpredictable resorption which sometimes leads to almost total resorption in between implants.I think today with all the alternatives to autogenous using hip graft for sinuses is contraindicated.
Hip grafting for sinus is overkill and I am an oral surgeon
I cant believe there are people doing that. I use a xenograft works awesome without invasive surgery of taking a hip.

Absolutely agree with the above comments, there is no need to perform an invasive hip surgery and take a patient to the OR for sinus augmentation. With the advent of infuse and the strong studies we have with xenografts and allografts why would anyone harvest a hip for sinus surgery.

Regarding the hip graft, complete overkill for a sinus lift in my opinion. In fact, I personally think most autogenous grafts are overkill. Very bold statement, I know. However, with modern growth factors, allografts, xenografts, and specialized surgical techniques, there are really very few defects that truly require an autogenous graft.
Open sinus lifts...just about anything will grow bone.
Narrow ridges...ridge split, block allograft, tenting screws, Ti-Mesh, Ti backed ePTFE, various bone putties. All work well.
The lit is beginning to show that autogenous is not always the gold standard anymore.

Dr. Holtzclaw, I could not agree with you more! When you consider the cost, morbidity and outcome, you have to ask yourself............... Why?
Hip Grafts for Sinus lifts is silly but may be needed elsewhere.
Look I think there is a place for everything and in some cases where full mouth reconstruction is needed a hip graft may be needed. With so many materials to choose from with proven research it is not needed to harvest a patient hip to do a sinus lift.
I mayself favor bio-oss or FDBA. Works fine without the complications of a OR case.

Hip Grafting: Limiting these days
I usually save this for extreme jaw recon's. I dont see the advantages of taking a hip anymore with so many products that can achieve similiar goals.

Hip graft for a sinus lift? Borders on malpractice IMHO.
With all of the lit on sinus lifts, there is absolutely no reason to go to the hip for bone. None! In my opinion, this borders on malpractice! When I bring this point up to "hipsters", they can never cite any lit to back up their position.

When you are in the show biz or any work that requires you to have interaction with clients in business looks do matter and tough many people wonder why people go in for cosmetic surgery I think in today's world its a truth that we have created for ourselves.

Hip Grafting: This should always be considered when needed.
Bone grafts from the hip for augmentation vary widely in results for several reasons. When travecular bone is used as the graft, resorption of 50% or more can be observed during the first 6 months. Therefore a barrier membrane should be used maintain the space. Cortical grafts from a femur resorb much less and volume reductions of 10% to 20% are common, although one quarter of the time less than 10% is observed.
The harvested bone takes more than 1 year to become mature bone. Re-entry at 2 months is too early- it alters the blood supply are a critical time to the graft, the implant surgery does not stop the resorption (it will continue and the implant may become compromised) and a cortical graft may become destroyed during implant insertion.
Re-entry at 6 months is optimal, although experienced clinicians may be able to use a 4 month time frame. At 6 months most of the resorption is complete, so you can evaluate if additional augmentation is needed before or during the implant insertation is required and although the graft is not all mature bone, enough bone has turned over to insure the process will continue. The implants are usually allowed to integrate for 6 months which means its host bone is in place 1 year prior to loading. Since regrafting is possible at implant insertion, an experienced surgeon familiar with augmenting procedures is suggested for implant insertion. If not, a CT and longer time frames are beneficial.
Hip Grafting: Indications for use
The only time I send someone to an oral surgeon for a hip is if that need both maxillary and mandibular augmantation both vertical and horizontal where I need a massive amount of bone.
Otherwise I take from a bottle.
Hip Grafting only needed for Full Mouth Recons
Why would anyone do a hip graft for simple augmentation especially a sinus lift. Buy some bio-oss alot cheaper and less invasive to the patient. You will probably get better results.
Hip grafts are only needed in severe resorption cases.

Hip grafting I use for a variety of cases. You need OMFS for this. Dont take this out of your bag of tricks
