How long do you wait post bone grafting for Implant placement?
Sat, 06/20/2009 - 15:12 — Arroyodds
I am a General dentist and I dont place my own implants and refer out. My question is I notice that there are patients of mine that will return from the surgeon with implant placed as early as 8 weeks post bone graft and as long as 9 months post bone graft. Why the variable time frame?

Implant placement post bone grafting- its a case by case basis
These are guidelines but I may wait shorter or longer depending on the case specifics.
Socket graft 2 to 3 months. GBR 6 months, open sinus lift 4-6 months, Block grafting 4 months, and a hip graft 3-4 months.
Wait time for Stage I post grafting
Here is my guidelines:
1) GBR 6 months
2) open sinus lift 4-7 months
3) Block 4-5 months
4) socket graft 3 months.
5) Hip grafts 4 monthts place and then load at 3 Months.
Wait time for implants after grafting.
1) GBR 4-6 months
2) open sinus lift 4-6 months
3) Block 4-5 months
4) socket graft 3 to 4 months.
Those sinus lift timeframes sound a bit long
I have never waited longer than 3 months (unless the patient missed an appointment) and I have yet to have any problems. The sinus is one of the most user friendly sites to grow bone. 7 months for a block graft, sure, but not necessary for sinus lifts.
Graft and time
I have found that with a particulate graft such as with Osteogen, 8 months produces optimal results. You may proceed as early as 6 mo. This is dependent upon the patients physiology, age, size of defect, # walls etc. I tend to stay away from human and xenograft products for various reasons. If you add PRP or PRF then you will get a jump on the turnover.
Wait time for placing implant post grafting
Waiting time having many vary variables which include size of original defect, the technique used, the type of bone used, experience of the surgeon, healing compacity of the patient (systemic disease)etc.
In General, GBR 4-6 months, open sinus lift 4-9 months, Block 4-5 months, socket graft 12 weeks to 4 months.
These is a basic outline
3-4 months max wait for me
No matter what grafting technique I do, I do not wait any longer than 3-4 months. Been doing this routine for years and have never had any issues.
GBR wait time will vary depending on case
You are correct. GBR waiting time will vary depending on many variables such as the original defect, the technique used to correct the defect, the type of bone used, experience of the surgeon, etc.
For example, if you are trying to grow bone laterally with traditional GBR (say with a tenting screw and particulate) you will need to wait quite a while and your resultant bone is going to be a bit soft. If you do a ridge split, your wait time will be less and the bone will be much more dense. If you do a block graft, your wait time will vary depending on the surgeon.
If you are simply grafting a socket, you are talking about a whole different and much easier ball game. The wait time here will be much less.
