I been using a piezo for some time on extractions. I have had some issue with irrigating the site as I feel like I cook the bone a little. Any recommendation?
Zero bone loss with an extraction. I definetly remove teeth a faster and does not remove bone. Most of the teeth that I remove are done flapless because of the powertome. There is less post-op pain and swelling for the patient.
I have used periotomes and luxators in my practice. Recently I bought a osada piezo unit and it makes my extractions easier. I have not used the powertome to compare.
I have both Mectron Piezosurgery unit and Westport Powertome. I have extracted teeth with both. The Powertome is basically just an electromechanical jackhammer with a periotome at the tip. It works great, but is difficult to get into posterior areas (requires straight line access). The Piezosurgery unit with the angled periotome-like tips is easier to get into the posterior, but I have noticed it "burns" the bone somewhat b/c irrigation saline and the tip cannot simultaneously get into the PDL space. The piezosurgery unit is a true piezotome, so can be used for other things like sinus lifts, any bone cutting application where protection of soft tissues is desired, whereas the powertome is only a power periotome.
I find the powertome useful...I usually use a mallet with my periotome, and the powertome is less stimulating to the patient but gives the same results.

I have not used the powertome although I read the White article and it looks interesting.
I been using the Mectron unit for some time. In addition to extractions I do sinus lifts and bone grafting with it.
I find my cases heal better with the piezo less trauma to the site. You have to avoid overheating the bone.

I own both. Hands down I am doing 90% of my extractions with the powertome. I use the piezo for sinus lifts and block grafting.
