Sinus Membrane Perforation: How Long to wait to go and relift?
Thu, 06/03/2010 - 12:35 — Oliver Queen DDS
This is a debate I am having with my residents and wanted to pose here. If the sinus membrane perforation is two large that you cant graft and you close up shop. How long do you wait before you can relift the sinus? How large does the perforation have to be to consider not proceeding with the case?

Sinus Membrane tear: Repair with a membrane dont suture
Sinus Membrane tear: Repair with a membrane dont suture. If it is large greater then a quarter I would close put a membrane over the window, and come back 4 months later.
Ted.
Sinus Membrane Perf: Why abort, suture it
I have sutured membrane perfs with very fine sutures. This will make it amendable to still graft the area and not have to abort.

Sinus Membrane Perf: When to go back in? Ziv what do you do?
I rarely abort the procedure. I think you can adequately repair a membrane and graft as oppose to aborting. If I do abort I would wait 6-8 Months.
Ziv you being the Sinus expert what do you do if you have a large tear? How long do you wait before you go back and relift?
Sinus Membrane Perf
I rarely abort the procedure even in large tears. My preferred method is the loma linda pouch.I graft the pouch and cover it with PRF.6 months later implants are inserted.
In case of abortion I wait for 5-6 months.
Sinus Membrane Perf with Balloon
Ziv do you ever get an perfs with the balloons? How do you know it is perfed and how do you handle it with a small preperation hole?
Ted
Sinus membrane Perforation Failed Bone Grafts
Give it about 4 months to repair, then reenter. Considering the big picture, implant dentistry is going down a narrow road. If more doctors would take the time to elevate their skills and learn how to place and restore blade and subperiosteal implants, they would not have to worry about sinus rears or failed bone grafts. The patients would win because the cost of and time for treatment would decrease. Granted a lot of thought has gone into root form treatment and it by and large is successful. Many of you out there reading this do not understand what I am saying. You may think you do but many just have not had the experience and keep on doing what the manufacturers and academics tell you to do. "I WOULD RATHER HAVE A MIND OPEN WITH WONDER THAN CLOSED BY BELIEF."
Sinus Membrane Tear: Wait 4 months
At our residency program we have consistenly waited 4 months after a sinus tear and gone back and the membrane is repaired and thicker to relift.
Sinus membrane Tear: Wait 3 months Max
IMO wait 3 months max. I think that is adequate enough to go back and relift. I rarely will close a case. The sinus tear has to be large enough or pathology present for me to close.
Sinus Membrane Perforation: I wait 9 Months to a year
You have to let the membrane before you go back or you will perf again.
Sinus Membrane Perforation: How long do you wait to reopen?
I usully never close the case. The tear of the membrane has to be fairly large for me to close. If it is super large I will close and return 6 months later and relift the sinus. It is usually thicker then.
