Zygoma Implants vs Traditional Sinus lifts and implants

A patient recently came to my office wanting to get rid of her dentures.
Which is a better solution for the upper arch: A sinus lift with 8 implants or bilateral zygoma implant with 4 implants?
What are the risks of zygoma implants?
Is there long term studies showing success?
How is the sinus adversly affected by putting the implant through the sinus?

I did one case of zygoma implants. The patient has constant sinus issues. Never again.

Zygoma implants vs Sinus lifting

Zygoma implants vs Sinus lifting. I dont see the benefit at all to a zygoma implant. Traditional sinus lifts and implant placement is greater then 95% successful with few complications.
Little is known about zygoma implants.

Lets look at a few considerations:
1) patient comes in with a floating upper and leaves with a fixed provisional - does not have to wear removable denture for an additional 12 months (6 for sinus and 6 for the implant integration!)That is a lot of tissue conditioners as well and a frustrated patient.Total Tx time from start to completion is 6 months + prosthodontic tx = about 6 to 8 weeks.
2)classic protocol is 4 anterior regular implants and 2 Zs / modified protocol is 2 anterior regular and 2 Zs VS 4 on each side with large maxilla or 3 on each side with small one. Fewer implants.
3) If you check the literature, the success rate for sinus vs Z protocol is about the same - 95 to 98 %.
4) Personal expeience over 30 years is that I have had very few problems with the Zs vs the lifts since many of the cocktail bone combinations don't always remain stable over time.
5) Completed more than a dozen Zs and have had NO failures - and have had no sinus issues. You have to have a very meticulous surgeon with lots of finese.
6) Contrary to what you state - there is lots that is know and reported about the Z implant protocol. Do a literature search and read the articles - there are many of them - As I indicated the % sucess is as good or better that lifts.
7) Not everyone is a candidate - all on 4 works just as good if you have bone in class 1 and class 2 areas. If there is no bone in those locations, then its a Z case.
8) The truth is out there and its in the literature of peer reviewed journals.
